Thank you for your interest in enrolling your child(ren) into the Stryker Local Schools in Stryker, Ohio. Stryker Schools has implemented an online registration system for all new students. Below are the instructions for completing student registration.

Please register at: https://stryker-oh.finalforms.com/ and follow the prompts to create your account, create your students account and sign your forms.

Be sure to click the “New to District” checkbox when registering your student.

If you require any support during the process, scroll to the page bottom and click “Use Support”.

Note: 
In addition to the online registration, parents must make an appointment and provide the following documents to complete the registration process:
- Birth certificate
- Proof of residency - utility bill, rental agreement, or mortgage statement with       parent's name and address
- Immunization record
- Custody papers (if applicable)
